Which patients should be part of the screening log?

A screening log record should be
created for ALL patients of participating study surgeons who meet each of the THREE
following criteria:
(1) are age 50-84 years,
(2) present with shoulder pain, and
(3) are not being seen for a post-operative follow-up appointment
