Clinic schedules should be pre-screened in advance, if possible, and a screening log record created for every person who meets the three criteria in A1 above. Ideally this should be done 1-2 days before the patient is scheduled to be seen in clinic. In rare circumstances when it is not possible to create a record in advance, screening log records should be entered within 1-2 days (no more than 1 week) after clinic. Every patient meeting the pre-screening criteria – regardless of whether actually evaluated in clinic or found to be eligible – should be included in the screening log.
