These are areas for you record any relevant information not already captured in the form that would be helpful for coordinating center staff or someone looking at the record later on to know (e.g., if the patient cancelled their original appointment and has rescheduled for another date, or the patient is leaving on vacation and will not be able to start PT or have surgery for another month, etc.) or to document certain changes or corrections made (e.g., when updating the info re: a rescheduled appointment, or if the MD completed the screening form incorrectly and a correction was made by you, etc.).
